The website was doing its job. Forms were being submitted. What happened after submission was the problem.
What was disconnected
Capture, response and reporting were three separate habits held by different people, none of them recorded anywhere shared.

Three of nine layers were in scope. Traffic sources and the website message were checked and left as they were — the constraint was entirely after the form.
What improved
Structurally: every enquiry now has an owner, a status and a timestamp; the first response is automatic and the human follow-up is prompted, not remembered; and marketing can see which sources produce enquiries that progress.
